Day Rehabilitation Without Walls Supervisor

Position: Day habilitation without Walls Supervisor
Reports to: Director of Adult Day Services
Hours of Work: 40 Hours Per Week

Key responsibilities:

  • Responsible for the direct supervision and training of the
    day habilitation staff.
  • Responsible to for the accurate and timely submission of
    billing and program data.
  • Responsible for the enrollment of people in need of day
    habilitation program
  • Responsible to develop individual day habilitation plans
    and chair elated meetings
  • Responsible for survey compliance and limited fiscal
    reviews outcomes.
  • Work closely with our compliance and records management
    staff in ensuring that all records are maintained with high integrity

General duties

  • Interface with community resources and ensuring that our
    consumers have ease of access
  • Develop strategies for community participation and inclusion
  • Work with ensuring that the musical theme of the program is
    fully engaged
  • Ensure transportation and logistics supports for all
    consumers.
  • Providing counseling services to clients, parents and other
    family member and assists them in coping with their situation and
    provides advocacy services to clients and their families.
  • Liaison between client and other agencies along with other
    community services and participate in inter-disciplinary treatment team
    process.
  • Safeguard the human and civil rights of the developmentally
    disabled person and their families.
  • Have the ability to formulate plans, implement strategies,
    provide therapeutic interventions, and be able to advocate for the
    clients and their families.
  • Have a working knowledge of the basic theory and principle
    of social work, with special skill in social casework. He/she should be
    abreast of new trends and developments within the field and have the
    capacity to apply these to practical situations.

Educational Requirements:
A Bachelor’s Degree from an Accredited College or University and a minimum of three years of experience as a treatment coordinator. Must have a valid NYC driver’s license. Master degree preferred in social services related area with 1 year of experience with developmental disabilities.
