Caseworker/QMR
Position: Caseworker/QMRP
Reports to: Director of Family and Social Services
Hours of Work: 40 Hours Per Week
Work Location: Tarrytown, NY
General Duties:
- Chair all ITT meetings and Special ITT meetings for 65
consumers - Write annual and semi annual and special ITT meeting minutes
- Be the primary contact for parents and other providers such
as other residences, transportation and medical facilities - Responsible for maintaining written correspondence with all
of our stakeholders - Research all related entitlements issues and work with the
entitlement specialist in resolving them - Provide advocacy for all consumers
- Work closely with our compliance department in ensuring
that our records are maintain in continuous survey ready state - Ensure that all related clinical activities are engaged in
manner consistent with HIPAA and privacy regulations. - Provide training in client rights and abuse prevention
- Responsible to provide administrative support during all
intakes and discharges. - Arranges appropriate client and parent visits, obtains
appropriate signatures for release waivers and obtains individual and
family histories. - Providing counseling services to clients, parents and other
family member and assists them in coping with their situation - Liaison between consumer and other agencies along with
other
community services and participate in inter-disciplinary treatment team
process. - Safeguard the human and civil rights of the developmentally
disabled person and their families.
Educational Requirements:
A Bachelors Degree from an Accredited College or University in a social
sciences area and a minimum of three years experience with people with
developmental disabilities.
