Caseworker/QMR

Position: Caseworker/QMRP
Reports to: Director of Family and Social Services
Hours of Work: 40 Hours Per Week
Work Location: Tarrytown, NY

General Duties:

  • Chair all ITT meetings and Special ITT meetings for 65
    consumers
  • Write annual and semi annual and special ITT meeting minutes
  • Be the primary contact for parents and other providers such
    as other residences, transportation and medical facilities
  • Responsible for maintaining written correspondence with all
    of our stakeholders
  • Research all related entitlements issues and work with the
    entitlement specialist in resolving them
  • Provide advocacy for all consumers
  • Work closely with our compliance department in ensuring
    that our records are maintain in continuous survey ready state
  • Ensure that all related clinical activities are engaged in
    manner consistent with HIPAA and privacy regulations.
  • Provide training in client rights and abuse prevention
  • Responsible to provide administrative support during all
    intakes and discharges.
  • Arranges appropriate client and parent visits, obtains
    appropriate signatures for release waivers and obtains individual and
    family histories.
  • Providing counseling services to clients, parents and other
    family member and assists them in coping with their situation
  • Liaison between consumer and other agencies along with
    other
    community services and participate in inter-disciplinary treatment team
    process.
  • Safeguard the human and civil rights of the developmentally
    disabled person and their families.

Educational Requirements:

A Bachelors Degree from an Accredited College or University in a social
sciences area and a minimum of three years experience with people with
developmental disabilities.
